After booking, you will receive an email invite to create your personal account, where you can fill out the necessary paperwork prior to your appointment in order to skip the check-in process and streamline your office visit.
Through the portal and app, you can also view upcoming appointments, consent forms, instructions, pay online, and more!
SCHEDULING
Please contact the office by phone 808-636-9084 or by filling out the contact us form to schedule an appointment. For a first time consultation with Dr. Tan, the fee is $150, and is collected when your come in for your appointment.
WHAT TO EXPECT
The initial consultation with Dr. Tan is meant to be an educational session. Any questions or concerns are answered in easy-to-understand, non-medical terms by Dr. Tan. An educated patient is better able to make a decision and has a more satisfying outcome. At MD Skin Lab, we want to establish a long-lasting relationship. built on trust and excellent results. Please expect to spend approximately 30min-1 hour in the office for your consultation. Arriving on time for your appointment ensures that Dr. Tan will be able to spend the full time with you.
Our practice has grown because of our patients’ kind referrals. In fact, our best patients are referred by people like you. We encourage you to recommend our office to your family and friends. We always have time to care for a new patient.
PRIVACY
Understanding the need for discretion and privacy, we strictly follow all rules of patient-doctor confidentiality and
federal standards for privacy under HIPAA (Health Insurance Portability & Accountability Act of 1996). We also ask that you show discretion and refrain from discussing other clients during your visit to MD Skin Lab. As a courtesy to other clients in the office, we ask that you refrain from cell phone use and turn off all phones upon entering MD Skin Lab. Our office has a discreet doorway, in addition to the main
entrance, to help maintain patient privacy. This is particularly helpful for those who have recently undergone a treatment or

REFUND POLICY
At MD SKIN LAB, we work with each client to discuss treatment objectives and review likely outcomes, benefits and risks associated with each treatment. We offer individual and customized treatment so each client may choose the approach that is best suited for their needs and budget.
Once services are purchased they will not be refunded, however, to ensure our clients always receive the greatest experience at MD SKIN LAB, unused service values (cash equivalent for the remaining amount of a treatment package) can be applied to any other service.
For Skin Care Products, all sales are final, however, should you have a skin reaction to one of the products, it can be returned for a full refund within 7 days of purchase.
All injectable treatment sales (such as, but not limited to Botox, dermal fillers, laser treatments, or chemical peels) are final; refunds or credits cannot be offered once treatment is completed.
We will be honest in all our dealings with you. Aesthetics is not an exact science and how you may respond to a given treatment will vary from person to person. It is virtually impossible to predict results and therefore payments made for services are for treatments to be performed — not for a specific result. However, we always strive to achieve the absolute best result that we can for you.
